This form has been developed to allow citizens to report
incidents of cruelty to animals. We understand that under certain
circumstance citizens may opt not to report such incidents due to fear
of retaliation, harassment, physical harm, etc.
Although this form allows any citizen to report incidents anonymously it
is strongly suggested that your contact information be provided.
Making yourself available for re-contact will allow us to ask any
questions that may not have been initially answered in your report, as
well as discuss how in some situations sworn written statements may be
used to support a criminal prosecution. Please word your report
carefully as it will be an official record.
Your information will be safeguarded, it will not be released, sold, or
distributed in any manner to a third party without your consent.
Using this form to report information known to be false, deliberately
misleading, or otherwise not true as well as to facilitate harassment is
a crime. Abuse will not be tolerated.
